hw=footway has default foot=yes. This can of cause be overridden by
explicite access tags.

access=yes (or permissive) is simply an abbrevation for (foot=yes &
bicycle=yes & motorcar=yes & horse=yes, and so on).

If you want to be less strict than default mkgmap you can add a style rule.
