Sorry, I have no tool to generate a patch file which  contains new files.
You have to apply the contours_v4.patch with the patch command (patch < 
contours_v4.patch) and copy the Java files by hand (HGTDEM.java to 
src/uk/me/parabola/mkgmap/reader/dem and GeoTiffDEM.java to 
src/uk/me/parabola/mkgmap/reader/dem/optional).
