More complete implementation of multiple RouteCenters.

Includes a rewrite of the handling of Table A indices. Arcs
now store both source and destination routes to be able to
determine when two arcs are inverse.

Writing Table A in the second pass moved to TableA.

RouteNodes now hold their full coordinates.
